Submitting a request early allows for optimal support through the potential implementation of accommodations for lectures and lab work from the start of the semester.
Deadlines for submitting an application for ESH status
To obtain EBS/ESH status, students must meet the following deadlines:
- During the first semester:
- Submit the application by October 15 (Part A via the online form)
- complete the application by October 31 (Part B, supporting documents)
- In the second semester: Submit the application by March 1; finalize the application by March 15.
Documents may be submitted by email or mail.
If you apply for EBS/ESH status, you may be eligible for a reduction in your annual course load.
Submit your application for ESH status
A complete application consists of a completed online form (Part A), to which you must attach the detailed medical report (Part B) completed by a doctor or any other supporting documentation (in a single PDF).
The required supporting documents vary depending on the disorder, disability, or illness (see detailed procedure).
For specific written language and math disorders, a comprehensive speech-language pathology assessment is required. These supporting documents must be recent (less than 2 years old at the time of application) and detailed. They must therefore confirm the diagnosis and explain its impact on studies or learning, justifying the implementation of accommodations.
Note: Wait times for appointments with a specialist or for an assessment can sometimes be long.
Procedure for Renewing the Individualized Education Plan (IEP)
This status is valid for the entire duration of the student’s studies. The Individualized Support Plan (PAI), signed by the student, the EBS/ESH coordinator, and the faculty coordinator, is valid only for the current academic year.
It must therefore be renewed each academic year with the CMP – Student with Disabilities Office by visiting during office hours or by sending an email.
To renew their PAI, students must meet the following deadlines:
- in the first semester: submit the request by 10/31
- in the second semester: submit the application by March 1
