BUMP Duplicate request - Student card Last name, First name UNamur identifier Student number UNamur e-mail address prenom.nom@student.unamur.be Reason for request Lost or broken card ( optional ) Wear and tear ( optional ) Lost or broken card: the card has been lost or is broken; Wear: one or more data on the card are unreadable Payment conditionsThe price of a duplicate following loss or when the card is broken is €10. In the case of wear and tear, the duplicate is free. The old card must be returned in exchange for the new one. Without the card, or if it is not so worn as to render one or more data illegible, you will be charged €10. In all cases, the duplicate must be paid for on collection from BUMP, using Bancontact/Visa/Mastercard only. An e-mail will be sent to you when it is ready for collection. I accept the conditions and agree to pay all related fees. In order to process your request, you must complete all fields marked "optional". When you submit this form, the completed data will be transmitted to UNamur and used to process your request.
