Last name, First name Postal address Street, number, postal code, city, country E-mail address Amount to be repaid IBAN Swift/BIC ( optional ) Repayment conditions I acknowledge having read the refund conditions ( optional ) As stated in the NetPay terms of use (available on unamur.mynetpay.be), the user may request a refund of their Credit Balance, as long as it is valid, for a €2.50 administration fee. The refund will be made to the account number communicated by the user in the "IBAN" field of this form. In order to process your request, you must complete all fields marked "optional".
